The Care Manager
This position is a supervisory role that will allow you to collaborate with our executive team and oversee our multidisciplinary care providers and ensure our clients receive the best home care and support services to meet their needs.
Meet with clients in-person for an assessment of their home care needs and available resources. Help clients to prioritize their health goals and develop an appropriate plan of care to meet their goals while remaining safely in their homes. Build and nurture a lasting relationship with clients and their families based on professionalism, trust, compassion, and caring. As the client’s advocate, you will act as a key point of contact for our clients and their families.
Coordinate and supervise an interdisciplinary team of schedulers, support workers, nurses, and allied health professionals to provide the highest quality and continuity of home care services.
Ensure the delivery of Home Care services in compliance with Aged Care Quality Standards.
Leverage our innovative and streamlined systems to provide the most efficient and effective delivery of homecare services.
